Pilates With a Clinical Approach
Clinical Pilates combines Pilates principles with an individualised, clinically informed approach to movement, exercise or an injury. Rather than following a standard class routine, each program is shaped around your health history, movement needs, current abilities, and personal goals.
At Spine & Pelvis Clinic, our clinical Pilates instructors work alongside our physiotherapists, combining Pilates-based exercise with clinical knowledge of the musculoskeletal system. This approach may be particularly valuable for people with specific movement concerns, those returning to exercise after an injury, building strength postnatally or women navigating changes to their body through different stages of life.

Could Clinical Pilates Be Right for You?
Clinical Pilates may be adapted to suit different ages, fitness levels, movement abilities, injuries and goals. You don’t need prior Pilates experience to get started, and your program can typically be modified to suit your individual needs. It may be suitable if you are:
- Managing back or spinal concerns: Working on strength, stability, and movement control around the spine.
- Experiencing hip or pelvic concerns: Aiming to develop strength, coordination, and control through the hips and pelvis.
- Navigating women’s health changes: Supporting movement and exercise through pregnancy, postpartum, perimenopause, menopause, and other life stages.
- Working on pelvic floor health: Incorporating appropriate exercises to work on pelvic floor awareness, strength, and relaxation.
- Work on Abdominal separation (diastasis recti) after a pregnancy by retraining the deep abdominal muscles and co ordinating with the pelvic floor
- Returning to exercise after an injury: Aiming to gradually rebuild strength, mobility, and confidence as part of an appropriate rehabilitation program.
- Living with persistent or complex pain: Exploring an individualised movement program that considers your symptoms, history, and exercise tolerance.
- Post breast surgery: can be particularly helpful to restore shoulder and chest mobility and rebuild upper body strength safely
- Looking to improve movement and posture: Aiming to develop greater awareness and control during everyday activities and exercise.
- Wanting to build strength and fitness: Use Pilates-based exercises to work on functional strength, stability, and movement control.
Clinical Pilates may also be used alongside physiotherapy, when appropriate for your individual circumstances.

Your Clinical Pilates Journey
While we have a focus on women’s health, the pelvis, hips, and spine, we can tailor your sessions to support a variety of conditions. Whether your goal is rehabilitation or injury recovery, building strength and stability, improving movement confidence, or maintaining an active lifestyle, we are here to help.
1. Initial Consultation and Assessment
Your first session begins with a conversation about your health history, current concerns, movement needs, and goals. Your trained instructor may assess your movement, strength, mobility, and control to understand where your program should begin.
2. Personalised Pilates Program
Your instructor will select Pilates-based exercises and equipment based on your assessment, current abilities, and goals. We may adapt exercises designed to accommodate your movement needs and may incorporate Pilates reformers, wonder chair, and other equipment available at the clinic.
3. Guided Progression
As your strength, control, and confidence develop, we can adjust and progress exercises as appropriate. Your program can evolve with you as your needs, goals, and activity levels change.

Your Clinical Pilates Questions Answered
What is clinical Pilates?
Clinical Pilates is an individualised form of Pilates that applies Pilates principles to a person’s specific movement needs, health history, and goals. Rather than following a standard class format, we select and adapt exercises to the individual, focusing on areas such as strength, stability, mobility, coordination, and movement control.
What is the difference between clinical Pilates and Regular Pilates?
The key difference is the level of individualisation and clinical consideration. Clinical Pilates is tailored to your movement needs, health history, injury, and goals rather than following a standard class routine. At Spine & Pelvis Clinic, our Pilates instructors also work alongside physiotherapists, considering your program within a broader understanding of musculoskeletal and women’s health.
Do I need to have done Pilates before?
No. Clinical Pilates may be adapted for people with different levels of experience, including those who are completely new to Pilates. Your instructor will select exercises appropriate to your current ability and can modify and progress them as suitable.
Is Clinical Pilates suitable during pregnancy?
Clinical Pilates may be appropriate during pregnancy, with exercises and positioning adapted according to your stage of pregnancy, individual circumstances, and movement needs. Your instructor can discuss your goals and help you determine the most appropriate approach.
Can clinical Pilates support women's health?
Clinical Pilates may form part of an individualised exercise program for women at different stages of life, including pregnancy, postpartum, perimenopause, and menopause. We aim to tailor sessions around areas such as the pelvis, stomach hips, spine, core muscles, and pelvic floor, depending on your individual needs.
Can clinical Pilates be used alongside Physiotherapy?
In most cases, yes. Clinical Pilates and physiotherapy often complement one another when appropriate. Physiotherapy may address specific symptoms, contributing factors, or rehabilitation needs, while clinical Pilates focuses on developing strength, stability, mobility, and movement control through guided exercise. Your physiotherapist or Pilates instructor can discuss whether combining the two approaches may suit your individual needs.
How often should I attend clinical Pilates?
How often we recommend you attend depends on your goals, current activity levels, and individual circumstances. Your instructor can discuss a suitable approach with you and adjust your program as your needs change.
Can clinical Pilates help with persistent or complex pain?
Clinical Pilates may be suitable for some people experiencing persistent or complex pain. We individualise sessions around your symptoms, movement tolerance, and goals, selecting and progressing exercises appropriately. If you are experiencing ongoing pain, an assessment may help determine whether clinical Pilates is appropriate and whether other forms of care may also be beneficial.
